Green Bay Sports Medicine - INSURANCE INFORMATION

Primary Medical Insurance

Green Bay Athletics and the National Collegiate Athletic Association (NCAA) requires that all student-athletes carry primary health insurance covering athletically related injuries and provide annual evidence of that health insurance. Proof of insurance is required before a student-athlete may participate in practice and/or competition for Green Bay Athletics. No student will be allowed to participate in any activity until such evidence of current insurance coverage is on file with Green Bay Sports Medicine. Proof of insurance for Green Bay Athletics requires that you provide the following information every year:

• Completing and signing the Student-Athlete Insurance Form, declaring that you have the appropriate insurance required by Green Bay Athletics and the NCAA

• A legible photocopy of both the front and back of your insurance card

Please notify Green Bay Sports Medicine with any changes that may occur to your insurance policy throughout the year. Any material changes or expiration of coverage should be submitted in writing to the Green Bay Sports Medicine office immediately after these changes occur.

Our Medical Insurance Information document provides further explanation of Green Bay Athletics' Insurance Rules.

Athletics' Excess Insurance Policy

The Athletic Department purchased an Excess Insurance Policy (EIP) for our student-athletes.  The EIP provides coverage for injuries occurring as a result of participating in sanctioned athletic activity.  Green Bay Sports Medicine will follow the provisions of the EIP when determining eligibility of a claim.  Illnesses, pre-existing conditions, routine medical care, or non-athletic injuries, will not be covered by the EIP.

Our EIP is offered on an "excess" basis to the participant’s required primary health insurance policy.  Under this provision, the plan will always pay secondary to any other valid and collectible health insurance plan for approved claims.  Therefore, in the event of an eligible injury, the treating provider will submit the medical claims directly to the primary health insurance plan first.  If there are any remaining balances, this amount is submitted to the EIP.  In a typical situation, an athletic trainer will provide the student-athlete with an insurance card to take to the health care provider indicating the EIP’s coverage.  For more information about our EIP, please review the Medical Insurance Information document or our EIP Frequently Asked Questions.

Please note that it remains Department policy that all student-athletes maintain their own primary health insurance.  Our  EIP will not be a substitute for any primary insurance!

Please contact Jeremy Cleven, MA, ATC, LAT at 920.465.2138 or clevenj@uwgb.edu with any questions or concerns about our Student-Athlete insurance policy.
